INSTRUCTIONS:
Only one entry to an envelope. Your handicap index must be 26.4 or less to be eligible. Players from clubs not sending a monthly handicap index sheet must play gross score for that day. Field is limited to 100 players. Entries must not be postmarked before Saturday, February 25th, 2012, and must be received by Wednesday, February 29th, 2012. Entries received after Wednesday, February 29th, 2012, will be accepted ONLY if the field is not full. The final cancellation date to have your entry fee returned is Thursday, March 8th, 2012. Please send entries with YOUR CLUB, YOUR FIRST NAME and HANDICAP INDEX on the bottom left corner of your check. Make sure there is also a contact number (cell or home) included on your check. Make check payable to C.T.W.G.A. for $52.00, which includes your golf, cart, and lunch.
