INSTRUCTIONS:
Only one entry to an envelope. Your handicap index must be 26.4 or less to be eligible. Players from clubs not sending a monthly handicap index sheet must play gross score for that day. Field is limited to 100 players. Entries must not be postmarked before Saturday, July 24, 2010, and must be received by Wednesday, July 28, 2010. Entries received after Wednesday, July 28, 2010, will be accepted ONLY if the field is not full. The final cancellation date to have your entry fee returned is Friday, August 6, 2010. Please send entries with YOUR CLUB, YOUR FIRST NAME and HANDICAP INDEX on the bottom left corner of your check. Make check payable to C.T.W.G.A. for $45.00, which includes your cart, continental breakfast, and lunch. Phone numbers must be on checks.

DIRECTIONS TO THE CLUB:
From Ft. Worth: take I-30 and exit on Cooper St. in Arlington. Left (north) on Cooper to first signal light, which is Lamar. Take a right (east) on Lamar, then the first left (north) into Rolling Hills.
From Dallas: take I-30 and exit on Cooper St. in Arlington. Stay to the right as you exit to Lamar. You will be heading east. Go through the light at Cooper and then the first left (north) into Rolling Hills.
